ED Case Manager

Provide case management services primarily for the Emergency Department on a full-time basis. Will also provide case management services to other departments as needed; will work rotating days.

Daily responsibilities include:
- Work closely with ED/admitting physician(s)
- Evaluate appropriateness of patients admitted to hospital
from ED
- Start case management worksheet on admitted patients
- Avoid inappropriate admissions
- Assist patients with discharge planning / social issues

REQUIREMENTS
RN with valid Texas license; experience in case management preferred, especially in ED and med-surg. Must have experience as RN, preferably in a hospital setting
