Primary responsibilities include:





Following up with mental health and substance abuse members that have been discharged from the hospital

Contacting members and providers to assist with positive treatment outcomes

Maintaining provider contacts and assisting care advocates to schedule appointments and services with providers

Working with community-based resources to obtain urgent car or treatment when necessary

Assisting with additional projects as necessary

Job Requirements:

Previous call center experience is required. A managed care background is preferred. MS Office proficiency is a must, as are solid typing skills. Mental health licensure is desirable. Knowledge of community-based resources is a plus.
