Blue Mountain Health System, a partnership between Palmerton Hospital and Gnaden Huetten Memorial Hospital, has an outstanding career opportunity for a Director of Case Management. The person selected for this opportunity will be responsible for the management and supervision of CM activities, insures timely chart reviews, discharge planning, communication with third party carriers, coordination of resources, and psychosocial support to patients, residents, and clients of BMHS. Qualified candidates possess a valid PA RN licensure, BS from an accredited school of social work or BS in Nursing preferred, 3-5 years case management experience required, and supervisory experience preferred.
