On-Site Case Manager Role Summary: Under administrative and clinical direction from the Case Management Team Leader, the Case Manager (CM) provides timely, clinically appropriate and cost effective case management interventions with focused utilization management activities for covered members within his/her jurisdiction. Care Management interventions are based on nationally recognized standards of case management practice. The Case Manager will develop effective working relationships with members, families and providers in order to: actively participate in the assessment of the member's needs and the development of individualized care plans matching the available Health Plan and community services to those needs; coordinate the ongoing evaluation of the care plan; recommend and facilitate adjustments to the care plan and services in place; focus on optimizing the member's/families level of independence in navigating the healthcare system at all levels of the continuum; and utilize opportunities to educate them on the Plans' focus of providing our member's quality, comprehensive and affordable health care coverage.Required License & Degree: BSN preferred. Registered Nurse with current unrestricted MA state license required. CCM is highly desirable.
