Continuous Care Scheduler

This position will schedule the activity of the Continuous Care Team. Assign qualified personnel to meet needs of patients. Provide scheduling information to team to provide Continuous Care. Provide excellent customer service. Travel as necessary



This position will

1. Schedule activities of Continuous Care staff. Responsible for scheduling and monitoring, staff hours/visits.

2. Oversee maintenance of patient/client clinical records, statistics, reports and records for purposes of evaluation and reporting of Continuous Care activities.

3. Assign qualified personnel to meet client needs. .

4. Assist the Patient Care Manager to develop, implement and evaluate the orientation program for new Continuous Care personnel.

5. Maintains employee assignment schedule and effectively communicates client information when assigning staff.

6. Assist with maintaining the Odyssey Quality Improvement Program.

7. Prepare reports on activities as required.

8. Participate in staff meetings, department meetings, team meetings, briefings, in-services, committees and other related activities as needed.

9. Communicate concerns of possible problems/situations with Patient Care Manager.

10. Respond to customer complaints regarding patient care.

11. Ensure Continuous Care hours are accurately entered into the billing system.

12. Maintain a log of recording the patients name, hours and employee that provided crisis care.

13. Participate in evening/weekend on-call or overtime as required, conducting on-call services in a responsive manner.

14. Regular attendance is to be maintained.

15. Comply with corporate policies and procedures

16. Assign caregivers utilizing negotiation skills to resolve issues

17. Maintain continuity of caregivers

18. Adherence to a code of conduct conducive with Odyssey HealthCare policy is expected.

19. Meet or exceed delivery of Company Service Standards in a consistent fashion.

20. Interact with all staff in a positive and motivational fashion supporting the Company’s mission.

21. Conduct all business activities in a professional and ethical manner.

22. Create and maintain a professional atmosphere that is also a fun place to work.

REQUIREMENTS


1. Minimum age requirement of 18.

2. High School diploma or GED

3. Hospice, home health or long term care experience preferred.

4. Knowledge of Medicare guidelines for continuous care and billing.

5. Compliance with accepted professional standards and practices.

6. Good physical stamina and mental health. Has presented a pre-employment physician’s health certification including a negative TB skin test and/or CXR and other tests as required by Odyssey policy and/or state law.

7. Valid driver’s license and reliable transportation that is insured in accordance with state and/or Odyssey requirements.

8. Satisfactory references from employers and/or professional peers.

9. Ability to work in an interdisciplinary setting.

10. Satisfactory criminal background check.

11. Demonstrate excellent observation and communication skills.

12. Flexible and cooperative in fulfilling all obligations.

13. Computer Literate

14. Excellent oral and written communication skills

15. Ability to prioritize, multi-task, and problem solve
