We are seeking an RN to help identify gaps in care for our valued members. As a Complex Case Manager, you would reach out to patients to ensure their healthcare needs are being met. Primary responsibilities will include:


Coordinating care and developing care plans


Aligning, coordinating and implementing the needed services/resources


Working closely with patients' families and physicians


Following up with patients to ensure they are following their care plans





Job Requirements:

The chosen candidate must be A Certified Case Manager (CCM) and an RN with a current, unrestricted license in the state of Georgia. Prior complex case management is ideal, and varied clinical experience is needed. Disease and catastrophic case management experience will be valuable. Computer skills are needed.
