Community Liaison and In-Home Aide

Hospice & Palliative Care Charlotte Region is
GROWING and as a result, is recruiting for the
following full time opportunities:

Community Liaison (Full Time) for our Referral Center (Admissions) Team - looking for a team player who is detail oriented with great assessment and critical thinking skills. Must have either a
Master's in Social Work from an accredited
program or a RN License.

In-Home Aide (PRN) for our Hospice Lake
Norman Team - Must be NA I (or II) and listed with NC DFS.
