Community Liaison

Join the professionals on our team and share in a quality career experience.The Community Clinic Liaison will collaborate with the community clinics to resolve any service issues; transition the underinsured, underserved population to the most appropriate level of service and clinic; and build programming to better meet the needs of this populations. Through the creation and building of these relationships with the community clinics and clients, the Liaison will be expected to promote the role of Covenant HealthCare as a community partner. The success of the Community Clinic Liaison interventions will be assessed by the following outcomes measured:

Emergency Department use and hospitalization pre- and post- intervention for patients transitioned to the most appropriate level of service and community clinics

Programs developed and implemented to address unmet needs

Communication plans developed and initiated for access and other problem resolution

Number of patients for which care was accessed and coordinated between clinic and hospital.

Satisfaction of the hospital and community clinic representatives, as well as clients, regarding the services provided.Job RequirementsBSN is required. Masters in Nursing is preferred. Must be licensed as a Wisconsin RN. Excellent skills in communication, collaboration, team building.Good understanding of levels of care/service provided by Covenant and community clinics.Must have automobile for use during working hours.
