Community Care Manager, RN (New Position, New Program!)

We are searching for an outgoing, personable RN to take on this brand new position. As a Community Care Manager, you would visit our valued members in their homes/assisted living facilities to ensure they are receiving the best possible care. Primary responsibilities include:




Communicating with members over the phone and in person


Assessing current care and developing a new care plan when needed


Interacting with primary care physicians, family members and home care providers


Helping members connect with the appropriate community resources and social service providers


Ensuring members are complying with their care plans


Job Requirements:

The chosen candidate must be an RN with a current, unrestricted license in the state of Ohio with substantial clinical nursing experience. Prior case management experience is needed; CCMs are encouraged to apply. A background in home care is highly preferred and community care management or disease management experience is ideal. A history in telephonic care is a big plus and excellent communication skills are essential. Computer proficiency is a must.
