Clinical Pharmacist - Home Infusion

The Home Infusion Department of Thomas Jefferson University Hospital has a full time position available for a Clinical Pharmacist. The selected individual will be responsible for maintaining high quality clinical services in the preparation and distribution of medications to patients at home. The staff pharmacist will provide necessary clinical services to patients, physicians, nurses, other pharmacist, as well as students.

Additional duties include:
Promoting rational drug therapy in the area of home care.
Monitoring drug therapy for appropriateness, drug effects, side effects, and toxicity.
Maintaining relationships with medical, nursing, social services, and other services involved in the care of patients at home.
Advising house staff and attending physicians regarding medications, and provide documentation of the same.
Advising nursing and other staff regarding medication administration, monitoring of drug therapy and disease state.
Interacting with home coordinators, home infusion nurse and pharmacists, physicians, patients, and other staff in identifying and assessing patients in preparation for home infusion therapy.
Serving as a resource individual for other professionals in various aspects of home infusion therapy.

Current PA Pharmacist License
