Beverly Living is seeking a Clinical Director!!! Exceptional career opportunity to influence the continuum of care for our long-term care skilled nursing facilities throughout the Southeastern WI region.

Responsibilities include:

·Assist referral sources in identifying patients with care needs that match services we provide

·Provide on-site clinical assessments to referring providers

·Relationship building throughout the professional healthcare community
REQUIREMENTS
Qualifications include:

* RN or LPN license is required

·Must have outstanding communication and documented assessment skills

·Ability and willingness to work on-site in numerous facilities

·Minimum of 2 years long-term care experience or hospital discharge planning experience. Preferred experience in case management as well as strong hospital discharge connections throughout the community.
