Clinical Coordinator-W. Valley

Essential Duties and Responsibilities:1.Provides and documents clinical and administrative supervision, consultation, and mentoring of daily clinical operations for Clinical Liaisons ensuring that the needs of consumers are being met and that services are delivered as outlined in each consumer’s assessment and Individual Service Plan.2.Brings to the attention of the Clinical Director and Administrator of the Clinic any unmet service needs identified by the Clinical Liaisons in the treatment planning process and assist Clinical Liaisons in identifying interim services available to help meet the needs of these consumers.3.Monitors and evaluates Clinical Liaisons’ (CL) performance based on job duties outlined in CL job description.4.Monitors productivity of CLs and CMs ensuring that minimum encounter values are met on a weekly basis.
5.Ensures that coordination of care is occurring with internal and external providers (e.g. probation officers, advocates, family member, hospital staff, jail staff, vocational rehabilitation, etc).
6.Monitors service delivery for the priority population to ensure that the behavioral health needs of these consumers are fully met. Prepares quarterly ASH 118 report and submits to Clinical Director.7.Monitors population on Court Ordered Treatment (civil and criminal) to ensure that consumer and team are adherent to all terms of the court order. Prepares monthly COT report and submits to clinical director. Ensures that CLs communicate with courts as required either in person or written report and that such communication is consistent with the assessments, Individual Service Plans, and team recommendations for the consumer.8.Works with the clinical team to ensure consumers receive appropriate care within the least restrictive setting including coaching and mentoring specific to inpatient and residential treatment discharge plans.9.Available on-call 24 hours/day, 7 days/week unless otherwise scheduled as part of a rotation or in event of a scheduled absence in which coverage has been arranged.
REQUIREMENTS
Minimum Qualifications:Education:Bachelor's degree in Behavioral Health or Related field.
Relevant Work Experience:Three years experience in the delivery of behavioral health services required.Two years experience in a SMI case management system preferred.
