Clinical Coordinator Exemption: Non-Exempt work schedule is Mon-Sat(Varies).Summary:
The Clinical Coordinator is responsible for the coordination of nursing care and services for the end stage renal disease patient.Qualifications:
1) Current RN license within state of practice.
2) One year of patient care experience. Minimum one year of dialysis experience in outpatient unit.3) Current CPR certification.4) Current basic EKG interpretation certification.Essential Job Functions:
Employment Management:1) Assist in the evaluation and counseling of facility staff under the guidance of the administrator.2) Participate in staff and patient scheduling and reports on staff performance.Patient Care:1) Perform patient assessment, vital signs noting presence of signs of trauma, patients' general physical and mental condition, review dialysis orders, documents observations in a legible manner.2) Assess patient and family learning readiness; and provide appropriate information on renal disease, prescribed dialysis treatment and medical regimen and their impact on the patient's state of health and well-being.3) Provide hemodialysis therapy; formulate a dialysis plan per physician orders and weight and rate to be ultra filtrated, ensure safety measures, monitor patient vital signs, administer medications or other appropriate treatments, discontinue dialysis according to established procedure, document all activity appropriately.4) Perform analysis of patient assessments, laboratory data, and, report results to physician if necessary.5) Participate in on going patient education.6) Administer patient quality care in a considerate, respectful manner.Planning, Coordinating, and Organizing:1) Supervise direct patient care.2) Review vital signs initiation and termination of dialysis, documentation, physical assessment of all patients each treatment.3) Ensure the transcription of all physicians’ orders is placed in patient chart.4) Ensure documentation of all laboratory work orders.Communication and Patient Service Skills:1) Inform administrator of all incidents, conditions, problems, counseling and the necessity for furthering counseling appropriately regarding staff, patient care, and facility management.Financial Managment / Quality Assurance:1) Ensure charges for dialysis are recorded according to company policy.2) Complete flow sheets and charge vouchers.3) Ensure availability of inventory and supplies.4) Participate in quality assurance cost containment programs.5) Compliance with DAVITA policy and procedures in accordance with state and federal regulations for patient care and services.6) Compliance with state and federal regulations of the Injury Prevention and Safety Training Program.Other Duties:1) Other duties as assigned.2) Able to work overtime with little or no notice.3) Ability to lift 35 pounds, and able to perform repetitive standing, sitting, stooping, walking, stretching, reaching and use full range of body motions.4) Understand and follow DAVITA patient and employment policy and procedure.DaVita Inc. is the largest provider of dialysis services in the United States for patients suffering from chronic kidney failure, also known as end stage renal disease, or ESRD.
