Clincal Coordinator/QMRP

The Clinical Coordinator/QMRP is responsible to develop an individualized annual plan for active treatment in an ICF setting. Will also coordinate assessments from various disciplines, chair team meetings, and develop and write behavior and skill programs. Will monitor data monthly and quarterly, provide training to direct care staff on individual programs and will coordinate any needed adjustments to the annual plan.
REQUIREMENTS
**Bachelors degree in Human Service or related field, MA preferred.
**At least 3 years experience in MR field
**Demonstrated understanding of positive approaches, person centered, active treatment and knowledge of the MR/DD population.
**Valid driver's licenseNHS is an EOE
