Under the direction of the U.D.O.N./R.N. coordinated the day-to-day functions of the nursing activities on assigned unit in accordance with the Evergreens policies and procedures and state and federal rules and guidelines.
Under the guidelines of the U.D.O.N/R.N. supervisor manage and coordinated the work performance of all Nurses Aides.
Administer medications in accordance with physicians’ orders and appropriate nursing care requirement.
Keep accurate records and nursing summaries on each resident.
Monitor use of supplies on assigned unit.
Establish effective communication and interaction with residents, their families and nursing staff.
Keep the UDON notified at all times of the status of assigned units nursing and medical care.
Major Accountabilities as it relates to the primary objectives:
Notify UDON of any changes in residents. Upon request notify physician and family.
Participate in determining shifts staffing requirement on assigned unit.Contribute in the development of the resident care plan.
Ensure that the Nursing Assistants are aware of the residents care plan and they are used in daily care.
Provide direct nursing care as necessary.
Report concerns and problem area to the shift supervisor, UDON/DON. Assist in developing and implementing corrective action.
Make frequent rounds to assigned unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
Meet with the C.N.A. on a regularly scheduled basis to assist in identifying and correcting problem areas, and/or the improvement of service.
Make periodic checks to assure that appropriate care is being properly administered by nursing assistants and to evaluate the residents’ physical and emotional status.
Make rounds with physician as necessary.
Notify supervisor when the resident is involved in an accident or incident. Chart incident and place on 24 hour nursing report.
Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressing/bandages, colostomy and drainage bags, taking blood if trained and certified.(As ordered by physician.)
Ensure that all nursing care is provided in privacy and that the nursing service personnel knock before entering the resident’s room.
Implements and monitors policies and procedures in accordance with Federal and State guidelines.
Assure an Infection Control Tracking Form is completed and forwarded to the Infection Control Nurse for all residents on antibiotics.
Participates in Fire and other emergency drills.
Understands Resident Rights and instructs staff to comply with these rights at all times.
Knowledge of Evergreens Policy and Procedure manuals at they pertain to the nursing department.
Monitor continually the use and release of restraints, hydration of each resident, adherence to Bladder and Bowel Programs, food intake, constipation, falls, etc. Keep supervisor aware at all times of any problems.
Maintain a written record of the resident’s complaints and/or grievances and make a written and oral report to the supervisor.Assist U.D.O.N. and R.N. supervisor in assignment of Nurse Aide. Guide and coordinate the work performances of all Nurses Aides working under his/her supervision along with the appropriate supervisor.
Assist U.D.O.N. and R.N. supervisor with the day-to-day evaluation of job performance of Nurse Aides on assigned unit. Assist with the performance evaluations program with recommendations for improvement and recognition for satisfactory performance.
Report immediately to R.N. supervisor any sign of verbal or physical by a nursing assistant, or reported by a resident.
Prepare and administer medication as ordered by the physician.Verify the identity of the resident before administering the medication.
Review MARs for completeness of information, accuracy in the transcription of physician orders, and adherence to stop order policy.
Review the residents’ chart for specific treatment, medication orders, diets, etc. as necessary.
Order prescribed medications, supplies and equipment as necessary, and in accordance with established policies.
Ensure that an adequate supply of floor stock medications, supplies and equipment are on hand to meet the nursing needs of the residents. Report needs to the supervisor.
Ensure that narcotic records are accurate for your shift. Count and sign record sheet at end of each shift. Report any discrepancies to the R.N. supervisor. Dispose of drugs and narcotics as required and in accordance with established policy.
Use the proper procedure for handling blood and body fluids and follow established procedures in the use and disposal of personal protective equipment.
Sign the resident's individual medication and treatment sheet when medications are given. Sign for all PRN medications with reason for giving and results.
Chart nurses’ notes in an informative and descriptive manner that reflects the care given to the resident as well as the residents response to the care.
Receive telephone orders from physicians and record on the physician order sheet. Fax copy of order to doctor for signature.Transcribe physicians’ orders to residents chart; medication sheet treatment care plans as required.
Notify others departments or orders, i.e. diet, physical therapy etc.
Review complaints and grievances made by the resident and make a written/oral report to the R.N. supervisor indicating what actions were taken to resolve the complaint or grievance.
Maintains chart on acute episodes for 72 hours including vital signs and pertinent information.
Assist U.D.O.N./supervisor in analyzing needs for assigned unit (linen, supplies, etc.)
Inform R.N. supervisor immediately when assigned personnel fail to report to work.
Provide leadership to nursing personnel, assigned to assigned unit/shift.
Communicate and listen to families and employees. Be objective and helpful. Review with UDON.
REQUIREMENTS
At a minimum must be a graduate of an approved Registered Nurse or Licensed Practical Nurse Program.
At a minimum must be an RN or LPN currently licensed to practice in the State of North Carolina.
