Casemanager
Position Objective: To provide casemanagement and training to program residents and to coordinate the implementation of program services.
Position Duties:
A. Conduct training in social and interpersonal skills, as needed.
B. Serves as an advocate on behalf of program residents.
C. Acts as liaison between residents, family, agency staff and other ancillary services.
D. Obtains documentation and maintains medical and program files of all individuals on the caseload.
E. assists direct care staff in delivery of program services.
F. Completes all financial record for individuals on the caseload and files appropriate reports with IDPA, DMHDD, and other funding agencies.
G. Assists House Manager and direct care staff with maintaining a safe and clean home environment.
H. Assists staff with intakes and terminations.
I. Documents all medical information including, but not limited to, hospital stays, medical appointments, dietary changes, medication regimen, and medical conditions, and notifies family/guardian where needed.
J. Monitors/disburses moneys from residents' personal accounts, maintaining appropriate records and distributing quarterly statements to guardians.
K. Schedules and conducts ISP meetings and other special staffings as needed and monitors individual status/progress for planning purposes.
L. Provides input to CRS Administrator and House Manager regarding direct care staffs' performance on job tasks.
M. Monitors and completes documentation pertaining to self-medication program.
N. Writes goals and objectives and monitors tracking for all individuals on the caseload.
O. Monitors residents for appropriate clothing, behavior, hygiene, and work patterns, taking corrective action where needed.
P. Maintains family contact log.
Q. Provide transportation for program residents using Agency vehicles or personal care as needed.
R. Ensures program compliance with all licensing/funding guidelines.
S. Keeps CRS Administrator informed of all relevant information.
T. Links residents with community agencies/services as recommended in the ISP. U. Remains available in emergencies through the us of a pager.
V. Solicits consumer satisfaction input from program residents and/or guardian and works to develop greater satisfaction.
W. Completes all required paperwork accurately and in a timely manner.
X. Completes other duties as assigned by supervisor.
REQUIREMENTS
Candidates must meet the Department of Human Services requirements for a Qualified Mental Health Professiona. (QMRP).
BA/BS in a human services field, including but not limited to sociology, special education, rehabilitation counseling, and psychology. Also must have one year of directly related experience, working with individuals with developmental disabilites.
Must have valid Illinois Drivers License in Good Standing.
