Case Mgr, Discharge Planning RN

Our client in the Towson area is need of an experienced RN Case Manager with discharge planning. This CM/UR Nurse will facilitate the delivery of quality, cost effective, patient centered care from pre-admission to post discharge. Will ensure that the care is designed to meet individualized patient outcomes. Must have current Md. Licensure and a minimum of five (5) years experience in heavy clinical environment with Med-Surge preferred. Also must possess the extreme desire to join a friendly,qualififed, cohesive team.
