Our Case Manager Social Workers are responsible for coordinating social and environmental needs of the AMERIGROUP member with physical medicine and behavioral health. She/he provides recommendations to members and significant others on accessing behavior health, caregiver services, community based organizations, safety programs, financial assistance and other social organizations.
REQUIREMENTS
Along with a current Texas SW license, the successful Case Manager, Social Worker must be a self starter with a demonstrated history of:

- Successful Managed Care experience,
- 3-5 years advanced experience in health care, long term care, case management or discharge planning,
- Strong assessment and patient interviewing skills,
- Handling complex clinical situations with emphasis on managed care,
- Creativity and the ability to think outside of the box, and
- Computer literacy
- Bachelor’s degree required, Master’s degree preferred.

Community health experience, Texas State TILE certification, and fluency in Spanish would be a plus.
