Case Manager, RN - Hospice / Home Care

We're searching for a compassionate, independently motivated RN to fulfill a Case Manager role based out of our Vienna, VA office. This role is autonomous, offers an opportunity to serve a needy and vulnerable population, and is replete with personal and professional rewards. Primary responsibilities include:


Providing case management and nursing care to hospice enrollees


Acting as a liaison between Evercare and nursing facilities


Educating staff, patients and families regarding end of life issues


Participating in interdisciplinary meetings

Job Requirements:

The chosen candidate must possess a current and unrestricted RN license for the state of VA as well as clinical experience in a hospital or other healthcare environment. Experience with hospice or home health would be ideal. Pain management skills are highly desirable. A background in oncology would be a huge plus. Solid computer proficiency is a must.
