Key responsibilities include:


Meeting with patients and monitoring their care plans


Working with physicians on a variety of case management efforts


Visiting patients in San Francisco, Alameda and Contra Costa counties to ensure their adherence to their treatment plan and to identify/resolve potential issues


Documenting all case management activities and providing continuous updates on patients in the system

Job Requirements:

Only licensed California RNs will be considered for this role. Solid healthcare experience is also needed. Long term care experience is preferred. Familiarity with Medicare/Medicaid would be very helpful in this position. Proficiency with MS Office applications is essential.
