Suburban Health Organization, a westside physician and hospital network serving central Indiana is seeking a Care Management Specialist with 2 to 5 years experience who will serve as a member of the Care Management Team responsible for coordinating and supporting the daily operations of the Care Management department.

The Care Management Specialist will intake and triage all Provider and/or Member requests entering the Care Management department and will resolve issues ensuring one call resolution whenever possible. This position will also assist the Care Management Programs as developed.

Qualified candidate will possess strong customer service, excellent problem solving and written/verbal communication skills. Candidate must have a basic knowledge of medical terminology and coding and the ability to adapt easily to change.
