Cummins Behavioral Health Systems, Inc. is currently looking for highly-motivated, innovative care coordinators to become an integral part of our high performance work team based out of our Montgomery County (Crawfordsville, IN) location.The care coordinators will provide services at our Alternative School program. These services include: group work, individual activities of daily outreach, in-home and community supervision to meet the comprehensive needs of youth with severe mental illness, addictions, and/or severe emotional disabilities.
REQUIREMENTS
Graduation from an accredited university with a minimum of a Bachelor’s Degree in a human services field and/or health related field, or a High School diploma or GED and a minimum of three years of direct care experience is required.Excellent organizational and communication skills are essential for these positions.An outstanding compensation and benefit package awaits those individuals chosen for these positions.
