Care Coordinator

Serve as the EPSDT/Care for Kids point-of-contact for referring physician offices and families from the Healthy Mental Development Initiative to link children to intervention services related to developmental health concerns.
REQUIREMENTS
After receiving a physician office referral, make contact with clients via phone and/or through home visits to assess all needs for successfully linking families to recommended services. Assist family with scheduling appointments, finding interpretation, childcare, transportation and other resources as necessary to access service. Must be able to communicate effectively, prefer experience working with diverse communities, and knowledge of community resources.
