Clinical Pharmacist/Specialist-Full time days, Mon-Fri,
Fawcett Memorial Hospital,
Port Charlotte FloridaThis clinical pharmacy specialist serves as a resource in the provision of pharmaceutical care to patients. The clinical pharmacy specialist will also:
Develop and implement new pharmacy practice programs and initiatives to further promote the safe and effective use of medications.Identify, prevent, assess, and resolve medication-related problems.Build effective relationships with other healthcare professionals and appropriate manufacturer representatives within their specialty.Promote clinically rational drug therapy and sound pharmaceutical care through the development of new (or expansion of existing) pharmacy practice programs, drug therapy policies, and other programs.

Education/Experience:
B.S. or Pharm.D. from an ACPE-accredited School of Pharmacy and a current pharmacist license in good standing with the appropriate State Board of Pharmacy
Completion of a 1-year pharmacy practice residency, specialty residency, and/or fellowship, or at least 3 years of pharmacy experience is strongly preferred
Certification by Board of Pharmaceutical Specialties is strongly preferred
Experience using computer applications including word processing, spreadsheets, and pharmacy information systems requiredKnowledge/Skills:
Thorough knowledge of state and federal laws and regulations governing pharmacy services
Proficient in the use and application of new medications and technology
Effective verbal and written communication skills are required
Ability to effectively participate on an interdisciplinary team or committee
Knowledge of pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics
Demonstrated ability to lead a team of pharmacists and other health care professionals to review, develop, implement and monitor clinical programs, therapeutic initiatives and other drug use policies and practices
