CASE MANAGER, MISSION CHILDREN'S CLINIC, FT DAYS

Works with appropriate Service Line to identify and implement best practice patterns across the continuum that provide optimal patient care, efficient resource utilization, and top decile performance for patient satisfaction and quality performance indicators. Maximizes positive financial outcomes for designated patient populations by intervening at key points in the disease process.
Required Bachelor's degree; preferred experience in Pediatrics, BSN, or Master's in Nursing or public health;
