Acute Registered Nurse (Northwest Indiana) - (MC29101-490-IN)

Acute Registered NurseExemption: Non-ExemptSummary: The Registered Nurse is responsible for providing acute, chronic or peritoneal dialysis therapy. Independently performs critical tasks in assessing, initiating, monitoring, and terminating treatment in an acute hospital setting.

Qualifications:
1) Licensed RN in the state of practice.
2) Two years experience preferred, in the nursing care of acutely ill patients (ICU/CCU experience preferred. CNN desirable).
3) Minimum of one year experience in chronic hemodialysis preferred.
4) Able to attend additional training on EKG interpretation, continuous ambulatory peritoneal dialysis and hemodialysis.
5) Current CPR certification.

Essential Job Functions:
Patient Care
1. Understand and follow established protocol per physician’s orders and procedures for patients in the hospital setting.
2. Perform patient assessment: vital signs and all other parameters of critically ill patients, review dialysis orders, determine general physician and mental condition of patient through observations, or review with physician and beside Nurse, assist in determining compliance to dietary or medication regimen, document observations appropriately.
3. Provide chronic and/or acute hemodialysis therapy or aphaeresis; formulate a dialysis plan for ultrafiltration, initiate dialysis via various accesses, observe all safety measures, monitor patient vital signs, administer medications, fluid therapy and blood products or other appropriate treatments as needed, document activity appropriately.
4. Perform analysis of patient data: assess and monitor status of vascular accesses, review lab work, patient assessment, and report any relevant changes or abnormalities to physician.
5. Instruct patient to all phases of treatment per physician's orders (include pre and post treatment)
6. Maintain effective channel of communication:
a. Obtain and giver pertinent report pre, during and post dialysis to the patient's nurse or charge nurse of assigned area.
b. Provide bedside teaching to the patient's assigned nurse regarding access care and/or variations of patient care in relation to renal problems, as necessary.
c. Inform attending physician and administrator of any unusual occurrences.

PLANNING, COORDINATING, and ORGANIZING:
1. Prepare and maintain dialysis machines: Prepare dialysis water treatment system for use according to established procedure, prepare dialysis delivery systems for use according to established procedure, and inform technical staff of any malfunctions. Prepare Peritoneal supplies per physician prescription and patient symptoms.
2. Clean and disinfect dialysis equipment per established protocol.

QUALITY ASSURANCE
1. Ensure accurate changes for dialysis according to hospital and company procedures.
2. Complete flow sheets and charge vouchers.
3. Order inventory and supplies. Ensure that the supply cart is fully stocked after each use.
4. Understand and follow DAVITA policy and procedures as related to patient care and employment.
5. Compliance with DAVITA policy and procedures in accordance with state and federal regulations for patient care and services. (Medicare, OSHA, or other governing agencies)
6. Compliance with state or federal regulations of the Injury Prevention and Safety Training Program.
7. Attend hospital mandatory in-service as required.

OTHER DUTIES
1. Other duties as assigned.
2. Able to work overtime with little or no notice.
3. Accept on-call time as scheduled to provide 24-hour nursing coverage.
4. Understand and follow DAVITA patient and employee policy and procedure.
5. Ability to lift minimum of 35 pounds, and able to perform repetitive standing, sitting, stooping, walking, stretching, reaching, and use full range of body motions.
