Salary:
$17.04 - $22.85 hr
• In addition to salary and vacation, 3 weeks of Compensation Credits may be exercised as time off with pay or as supplemental income.

required. Provides services in the office and in the field. ibility: Determines eligibility; reviews and responds to evaluation reports per Senior People with Disabilities (SPD)
requirements. Plans and Annual Summaries: Attends and participates in development of Individual Support Plans (ISP) for comprehensive,
in-home comprehensive and family support individuals; completes annual summary for all individuals not enrolled in any other
SPD anagement; participates in transition planning by attending Individual Education Plan (IEP) funded service, except case m
meetings for students age 16 or older.
Service Monitoring: Monitors services and supports for individuals enrolled in case management as described in OAR 411-
320-0130 and documents per SPD published guidelines.
Service Authorization: Authorizes services and supports as described in OAR 411-320-120.
Information and Referral: Provides information, timely referral and assistance to individuals regarding available services;
completes enrollment, termination and transfer on required forms.
Child Welfare: Coordinates and ensures provision of required supports from Community Developmental Disability Program
(CDDP) according to guidelines published by SPD.
Crisis: Assesses, identifies, plans, monitors and evaluates crises per OAR 411-320-0160.
Nursing Home Services: Facilitates referrals to nursing homes and notifies SPD of referral.
Medicaid Waiver: Ensures that Medicaid eligible individuals are offered the choice of home and community-based waiver
services; provides a notice of fair hearing rights and completes Title XIX Waiver form that is reviewed annual or as significant
changes occur.
Licensing and Certification: Ensures certification and licensing are completed according to SPD published guidelines.
BEHAVIORAL EXPECTATIONS:
Attends meetings; complies with Health Department behavioral expectations, county and department policies and procedures;
promotes and contributes to positive departmental relationships based upon the department’s mission, vision and values and
interest based decision making; uses technology tools, such as computers and other hardware and software programs, as adopted
by the county and the department to conduct and discharge department and service area business and to maintain, collect and
report client and service information; provides backup and support to co-workers; wears personal protective equipment when
performing job tasks that increase risk of exposure to blood or body fluids; establishes and maintains professional and effective
working relationships with customers, co-workers and other agencies; maintains punctual and regular attendance; complies with
department and county policies, procedures and regulations; participates fully as a member of a team; participates fully as
support to a service area(s); participates in employee training and orientation; provides training to co-workers as requested;
maintains licenses/credentials as required; performs other job duties as assigned by supervisor.
PHYSICAL REQUIREMENTS:
Operates a motor vehicle in the performance of duties; maintains a current Oregon driver’s license and an acceptable driving
record; sits up to ½ hour at a time; moves about the work area; bends forward; climbs 1 flight of stairs; lifts and carries up to 3
pounds; operates a keyboard; uses a clear and audible speaking voice; reads a 12 pt font; uses depth perception; hears a normal

REQUIREMENTS
MINIMUM QUALIFICATIONS: It is imperative that applicants include, on the application form, details of education, skills and experience as they pertain to the qualifications shown below.
1. Any satisfactory combination of five (5) years paraprofessional experience, training and/or education consisting of serving individuals with a primary diagnosis of developmental disabilities within a residential, employment or case management service at a management or case management level; AND
2. Knowledge of the public service system for developmental disability services in Oregon.
NECESSARY SPECIAL REQUIREMENTS:
• This position provides interpretation services to monolingual Spanish-speaking clients; therefore, fluent bilingual Spanish/English skills are required. An additional 5% will be added to the hourly wage as compensation for this skill. Finalist will be required to pass a Standardized Spanish Fluency Assessment as part of the hiring process.

Driver’s license number(s) and state(s) in which they were issued, during the last 5 years, must be included on the application form in order to qualify for the position.
