HOME HEALTH CLINICAL CASE MANAGEMENT NURSE (LPN)

Brentwood, TN based homecare network is seeking a clinical case management nurse to be responsible for the coordination and clinical management of home health care services.
Specific duties and responsibilities:
Communicate via telephone with physicians, insurers, and home health care providers to arrange and manage services
Perform administrative and clinical activities for individuals needing homecare, hospice, DME or home infusion services
Oversee other team members including Intake Specialists and Care CoordinatorsPosition will also include participation in on-call rotation.
REQUIREMENTS
Licensed Practical Nurse with current Tennessee license.3-5 years clinical experience with at least 1-2 years in case management required; Managed care experience with Medicare and/or TennCare preferred.Knowledge of ICD-9 Diagnostic coding, CPT Procedural coding, and length of stay guidelines helpfulStrong telephone and problem solving skills requiredBasic knowledge of medical claim system authorization modules required and computer skills in word processing and spreadsheets also helpfulCandidates must possess excellent written, verbal and customer service skills
