JOB TITLE:CASE MANAGER, Teen Parent Services
LOCATION:PLCCA – Maywood, ILDEPARTMENT:Youth & Family Services
Description: Position functions to assist with the implementation of objectives for the Illinois Department of Human Services/Youth & Family Services Program as well as facilitate the development and/or improvement of program’s referral network of agencies in the area that provide services to participants and their children.Duties:Include, but are not limited to, providing intake, outreach and comprehensive case management services to program participants; conducting interviews and Needs Assessments for program participants; providing referral/follow-up to program participants for needed resources; coordinating interdepartmental services and resources for participants; scheduling/coordinating home visits; coordinating with community agencies for information/referral; serving as liaison between participants and local DHS office; recruiting additional participants; assisting in the preparation of monthly reports; and maintaining case management files/records.Qualifications:Bachelor’s degree preferred, Social Work or related field plus a minimum of one (1) year experience in community social services; or 3-5 years experience working in a community social service agency. Strong interpersonal skills, good oral and written communication skills required. Knowledge of social and health services in Cook County.
