Service Facilitator

Service Facilitator Person needed to act as service and support administrator for County Board of MRDD. Service facilitator will assist individuals with MRDD and their families to achieve what is important to them, including assisting the individual to develop a person-centered plan and budget; choosing providers; performing assessments; coordinating and monitoring services; and providing crisis intervention as needed. Successful applicant will be a team player who is capable of managing multiple priorities. Must be computer literate; have a Bachelor's degree; be eligible for ODMRDD certification; and driver insurability. Flexible hours. Position will be based in NW region of Hamilton County.
