DIRECTOR OF PATIENT SERVICES
American Eldercare, Inc., a rapidly growing home care company with locations throughout Florida, offers you an excellent opportunity to use your home care management skills towards assisting patients recovery and rehabilitation of acute illnesses, or chronic disease.

STATEMENT OF RESPONSIBILITY:
Responsible for overall direction of home health clinical services. This individual directs and supervises the delivery of services, and implements and evaluates clinical outcomes.

FUNCTIONS:
Coordinates and oversees all direct and indirect patient/client services provided by clinical agency personnel. Serves as resource to all direct staff for individual patient plans of care or other related issues and concerns.
Reviews OASIS documentation and directs coding for accurate and appropriate services to Medicare patients; teaches nurses to properly conduct a comprehensive assessment.
Assists with hiring process for all direct care employees; orients professional and paraprofessional staff, as appropriate. Participates with agency staff development process.
Develops and implements in-service and staff development activities for clinical staff. Implements program for initial and annual review of skill competencies.
Assists the Administrator in development of the Annual Report, Annual Goals, and Annual Budget.
Maintains clinical record system that is consistent with regulations. Monitors implementation of policies for clinical records, intervening when necessary to provide ongoing consistency to standards.
Maintains infection control system, including surveillance process with follow- up for issues that surface.
Evaluates Clinical Supervisors and full time clinical employees; provides direction for performance evaluation of all clinical professional and paraprofessional employees.
Participates with Committees, including Professional Advisory, Performance Improvement, and other management meetings.
Implements and monitors policies and systems for provision of patient and employee safety such as: Emergency Plan, Home Safety Assessment, and On-Call System. Provides immediate report to Administrator when problems are identified.
Monitors patient-incident reports, complaints and/or grievances and provides investigation and follow-up when appropriate. Participates with process for reporting and follow-up of employee incidents.
Directs and/or participates with Performance Improvement process through quarterly clinical record review and/or OBQI/OBQM process activities.
Maintains effective on-call system for after hour services, providing supervision and support to on-call staff.

REQUIREMENTS
QUALIFICATIONS:
Registered Nurse, Licensed in the State of Florida

EXPERIENCE:
•Minimum of three years experience as a professional nurse.
•One year of supervisory experience in home health required.

KNOWLEDGE:
Familiar with Medicare home health and licensure regulations; Skilled with OASIS assessment; Basic knowledge of Quality Improvement process; Understands principles for infection control. Able to perform nursing skills and procedures.

OTHER:
Ability to manage multiple projects/responsibilities with ease; Communicates policy, procedures and standards to employees and oversees their implementation. Skilled with interpersonal relations and team building. Self directed, flexible in fulfilling obligations.
