Care Manager Trainee - LTCO
DEFINITION: Reporting to the Care Management Supervisor, this position is responsible for the overall care management of a diverse caseload. The care manager facilitates community-based long-term care to disabled consumers, age 18 or older, and/or chronically impaired older consumers, and/or their caregivers in their own residence. This position carries responsibility for ongoing care planning, service arrangement, follow-up, and reassessment. The care manager is a consumer advocate as well as the gatekeeper of service provision. The care manager trainee position has increased supervision by the Care Management Supervisor and Nurse Consultant.
GOALS:
To maintain high quality care management services by identifying and meeting the varied needs of PCA consumers. The care manager maintains the highest level of professional ethics, continually seeking opportunities for growth and development.
DUTIES AND FUNCTIONS:
a.Performs all care management functions. Validates the assessment of the consumer's needs in their environment. Reassesses consumer's status and reviews care plan at regularly scheduled intervals.
b.Works with consumer, his/her family, and/or caregiver, to develop a care plan, making use of supervision and consultation with other disciplines as necessary.
c.Recognizes, supports, develops, and mobilizes informal and formal resources to meet consumers' needs.
d.Arranges for needed services and entitlements, working cooperatively with consumer, family members, and service providers. Follows up on service delivered in specified amount of time, and works with consumer and provider to assure appropriate match of service to specific need.
e.Manages financial and/or resource allocations, matching services to specific needs to provide appropriate amount of service, while controlling costs.
f.Completes all necessary forms for consumers case record, other management information, and other written reports as required. Inputs necessary data and maintains automated programs.
g.Arranges case conferences with supervisory approval.
h.Participates in orientation and training, in-service training as assigned, and attends regularly scheduled supervisory, team and staff meetings.
i.Monitors consumer satisfaction to insure quality of services provided.
j.Collects, verifies, and documents consumer information required for annual recertification of consumers.
k.Facilitates clear and timely information sharing and linkages with the County Assistance office in the recertification of consumers.
l.Maintains and facilitates open communication with consumers/families and provides assistance as appropriate in the co-payment due notification process. Links consumers with provider-based financial management services where necessary.
m.Monitors Bridge Program consumer asset spend-down. Educates prospective Waiver consumers regarding features of the program and facilitates choices to transfer. Notifies County assistance Office of Bridge Program consumers nearing Waiver eligibility.
n.Other duties as assigned.

Experience:
?Less than two years social work or related experience.
Personal Characteristics:

?High level of flexibility.
?Possesses advocacy skills and a sense of professional ethics.
Additional Requirements:
?Must have valid driver's license, a good driving record, and continuous access to a fully insured car.
?Pre-employment physical.
?Drug testing.
?Criminal history clearance.
?Second language abilities preferred.
PERFORMANCE EXPECTATIONS:
?Meets agency standards of care management practice.
?Records and reports are legible, timely, accurate, complete, and relevant.
?Ability to establish rapport with PCA staff, consumers, providers, and outside agencies.
?Brings problems and possible resolutions to supervisory conference.
?Assures quality services to consumers.
