RN,Certified Diabetes Educator

Join Our Team!Responsible for developing and implementing the outpatient diabetes program for HealthCare Partners.Participates as a member of the interdisciplinary healthcare team to facilitate the continuum of care and education of patients with diabetes.
Monitors and evaluates, as needed, resource utilization, patient outcomes, patients' skills in self-care, ability/readiness to learn.
Counsels/educates individual patients appropriately on diabetes care in conjunction with their medical provider (medication adjustment, exercise, diet, sick days, etc.)
Evaluates blood glucose records for efficacy of current medication therapy, for problem solving of hypo/hyper-glycemia and for adjustment.
Ensures correct and timely documentation in the patients' medical record following an individual consultation.
Develops and conducts group presentations and related education programs as needed.
Develops and maintains patient education materials relating to diabetes.
Serves as a resource for clinic staff, home health, and other departments.
REQUIREMENTS
CA RN license.
Certification as a Certified Diabetes Educator.
Over 3 years and up to and including 5 years of experience.A bachelors degree from a four-year college and/or a professional certification requiring formal education beyond a two year college.Effective communication skills and teaching skills.
