Services include visiting nurses, rehabilitation therapists, medical social workers; home infusion therapy; home health aides, hospice care; bereavement programs; and community health clinics. Sutter VNA & Hospice serves ten Northern California counties with local offices in Emeryville, Concord, San Leandro, San Mateo, Santa Rosa, Sacramento, Tracy, and Roseville, and manages home health and hospice services in Marin County and San Francisco.
This position is located in Sacramento, California.
The QCE Liaison is responsible for coordinating and/or implementing the collection of quality and compliance-related data, which includes but is not limited to review of patient records and/or other clinically-related logs, identification and trending deficiencies, analysis of trended data, and instruction and inservice to designated staff. This individual also assists in monitoring performance improvement activities of the staff, as well as participated in orientation, education, and competency-assessment as assigned. REQUIREMENTS
Health Related Professional with Bachelor of Science Degree from an accredited school. Masters preferred. Course work in Quality Management and Statistics preferred. Must possess a current discipline specific California license and CPR certification. A minimum of two years in the disciplines professional field; minimum of one year experience in health care performance improvement and utilization review activities; and minimum of one year experience in clinical staff education activities preferred. In depth knowledge of Title XXII, JCAHO, and Medicare Home Health and Hospice regulations required.
