Health Educator
Job Description: We are looking for customer focused, high performing Health Educator to join and support our expanding health system. In addition to working in a dynamic and innovative environment, this candidate must provide excellent quality and care for our patients.
Responsibilities:
Functions as an experienced health educator in the provision of health management programs, education, and services to community members, responsible for design and coordination of programmatic activities impacting health and quality of life of Jackson are youth under the direction of the leadership of the Foundation for a Healthy Community.
Design, coordinate, and present Health Management Program materials for lectures and handouts to pediatric, adolescent, adult and geriatric clients.
Provides health risk appraisals, health screens and personal coaching sessions to program participants.
Assess client’s health history using a database cataloging health risks and recommends personal lifestyle changes to improve health using a comprehensive approach based on stages of change theory.
Develops and coordinates school-based assessments and intervention activities for a large project relating to youth health risk behavior in the specific areas of nutrtion, physical activity, obesity, and tobacco use. REQUIREMENTS
Requirements:
Master's of Public Health in Health Behavior/Health Education, Human Nutrition or related field preferred, or Bachelor's Degree in health related field with significant experience in community health program development, implementation and evaluation.
Strong verbal and written communications with commitment to a healthy lifestyle as a model.
Ability to identify existing community resources and collaborate with community organizations and health care systems to coordinate activities.
Demonstrated leadership and teaching ability as it relates to coordinated delivery of health education messages.
Experience in designing, coordinating , and presenting health management materials.
Demonstrated knowledge and experience applying behavioral change theory to health improvement in the areas of nutrition, fitness, and stress management.
