Direct Service Employee Job Description:
Under the direct supervision of the Coordinator, responsible for assisting individuals in accordance with the Individual Plan (IP) for the purpose of developing and improving basic life skills. Foster self-reliance and independent living for individuals receiving services. Position is full-time and/or part-time. Position accountable to the Coordinator.Also available is 1 Residential supervisor positionDirect Service Employee Daily Duties:
Provide for the delivery of services such as skills training, cognitive remediation, job coaching and behavior management in accordance with the Individual Plan (IP) and/or treatment plan.
Reinforce appropriate behavior and skills in residential, community and day/vocational settings, utilizing direct feedback, role-playing, modeling and behavior modification techniques.
Assist the Coordinator or other designated staff in the planning, scheduling, implementation and evaluations of social, emotional, recreational, vocational and related therapeutic programs for the individuals receiving services.
Provide input on scheduling to ensure adequate direct care coverage and assistance in emergency coverage.
Document contact with those receiving services and assists in the preparation of the treatment plan as well as other clinical and funding documentation as assigned. May participate in IP development/review meetings as requested.
Establish a relationship with the individual receiving services to ensure effective guidance, support and service delivery.
Provides training, assistance and/or supervision to individuals in the areas of daily living, personal care, financial management, community and social integration and/or job skills development.
Provides support to the individual to ensure objectives are met in accordance with the IP.
Coordinate, organize and/or assist with household activities such as light housekeeping, laundry and meal preparation.
Provide transportation for individuals receiving services to planned and/or necessary activities and appointments.
Maintains current progress and contact notes and any other appropriate documentation in accordance with MENTOR policy, program standards or other regulatory policies.
Complete and submit all required documentation to direct supervisor in accordance with established deadlines.
Provides direct supervisor with verbal feedback and data relevant to the individual's day to day programming, IP or administrative policy.
Actively attends and participates in multi-disciplinary team meetings as required.
Attends training and other meetings as required.
Provides emergency coverage an/or care for individuals receiving services.
Assist in the training of other Direct Care staff.
Assists with recurring or special projects as assigned.
Maintain confidentiality, respects human rights and practices universal precautions in accordance with MENTOR policy.
Assist individuals with medication administration.
Track all unapplied items for eventual disposition or application
Reconcile cash applied in system with cash log of all receipts
Additional Responsibilities as Needed:
Participate in strategic company initiatives such as worker's compensation breakthrough projects as requested.
Assist with special assignments or events in which a client may participate.

REQUIREMENTS
Direct Service Employee Applicant Requirements:Education: High School diploma or G.E.D.Experience: High School diploma plus three years experience or AA with one year experience -OR- Bachelors in related fieldCertification:Current driver's license in good standing.Car registration and proof of insurance.
CPR/First Aid certification.Knowledge, Skills, and Abilities:
Ability to deal with routine problems involving several concrete variables in standardized situations.
Ability to quickly grasp and learn new principles.Ability to work independently with minimal supervision.
Ability to apply common sense understanding to carry out instructions furnished in written or oral form.
Knowledge of the unique nature of the human services industry.

Knowledge of the unique nature of the human services industry.While performing the duties of this job, the employee is occasionally exposed to wet and/or humid conditions and outside weather conditions. Temperature extremes will be minimal to nonexistent.
The employee is regularly required to use hands to handle, feel and/or communicate. The employee frequently is required to stand, walk, reach with hands and arms, climb, balance, taste and smell. The employee is occasionally required to sit, stoop, bend, kneel, crouch or crawl. The employee is frequently required to hear and speak in order to use the telephone and communicate with people in a program service environment. The employee must lift and/or move up to 25 pounds.
The noise level in the work environment is usually moderate. The employee will be required to use general office supplies for documenting client information. The employee may occasionally be required to use a calculator, fax, copy machine, computer and direct inward dial phone system.Application Process:
