COMHAR, Inc. seeks a qualified individual for the position of a Children’s Case Manager in our Children’s Services Department’s EPSDT/BHRS (Early Periodic Screening Diagnosis and Treatment/Behavioral Health Rehabilitation Services) Program. This position offers services to children with serious emotional and mental health problems within their homes, schools and community. The Children’s Case Manager duties are as follows, but are not limited to:

•Assess service needs based on psychological evaluation
•Facilitate access to services
•Monitor effectiveness of services
•Identify, link, and coordinate services
•Maintain appropriate service logs which document all case management services provided to recipient
•Establish and maintain coordination with county Mental Health, Mental Retardation, CBH, and Children and Youth agencies, local schools and other child serving systems for those children who require services from other agencies
REQUIREMENTS
This position requires a BA/BS Degree in human services field plus one year experience in case management or counseling with children and families. Valid driver’s license required.
