About the Opportunity:
Do you have a genuine desire to play a key role in the life of another person who would benefit through your case management skills?
We are currently seeking, high energy, compassionate, motivated, flexible individuals who are self starters and have a real interest in working with individuals diagnosed with a developmental disability to work as SERVICE COORDINATORS.
Our Service Coordinators are responsible for developing, coordinating, implementing and monitoring the Individualized Service Plans (ISP) for individuals and their advocates. They provide case management services to all individuals on their assigned caseload. The duties include determining individual aptitudes, interests and preferences and then linking and referring the individual to appropriate service providers, activities, and developing natural supports for the individual. Service Coordinators are responsible for monitoring the individual’s satisfaction with their ISP and effecting a change in their ISP when required.

REQUIREMENTS
Experience and Skills Required:
This position requires a Bachelor degree, experience in case managment and/or social work background and working with people with developmental disabilities (MR/DD population) is a plus. Excellent English oral and written skills, the ability to interact effectively with coworkers, consumers, families and outside contacts and, good PC skills is a must! Bilingual skills are a plus.
