Outreach Workers/Case Managers

Leading non-profit organization is seeking full-time Outreach Workers/Case Managers to provide supportive services that enable to clients in Hamilton, Brown, Clermont and Clinton counties to obtain SSI benefits and guide them through the process of work readiness and barrier removal towards self dependency
REQUIREMENTS
EDUCATION: A Bachelor’s degree in social work or a related field with at least 3 to 5 years case management experience with adults with preference to experience with mental health, or Social Security background preferred
•Experience with the SSI/SSDI application process as well as community resources to address mental health and substance abuse issues.•Experience in providing case management / outreach casework services, including assessment, crisis intervention, emergency housing, case planning, and referrals for clients with multiple barriers•Must have good organizational, oral and written communication and interpersonal skills and able to maintain accurate casework files.
•Ability to work collaboratively with other social service agencies. Some knowledge of community resources is preferred.•Must have good organizational, oral and written communication and interpersonal skills.•Ability to effectively multi-task and problem solve successfully.•Proficient in all Microsoft software (Outlook, Word and Excel). Knowledge of the Crystal Reporting system as plus.•Requires travel to various sites in the Tri-State area, must have a valid Driver's license and automobile insurance.Downtown office space with interaction with the general public.
