Nurse Coordinator-Oncology Services-Full time

POSITION SUMMARY:
Takes exceptional care of people by coordinating the health care of select patient populations/designated service across the continuum of care using the nursing process. Promotes the safety and well-being of all patients and supports the key results area of Barnes-Jewish Hospital.

DEMONSTRATES JOB KNOWLEDGE:

-ASSESSES AND REASSESSES PATIENTS. Conducts age-appropriate assessments. Identifies physiological, psychological, teaching, learning and discharge needs in all assessments. Reflects safety, drug therapy, sensory, behavioral and developmental needs in all assessments. Collects complete and accurate data. Interprets data/test results accurately and timely.

-PLANS DELIVERY OF CARE. Involves patient and family in care decisions. Uses age-appropriate communication techniques
Determines and prioritizes patient's needs in collaboration with the patient and healthcare team. Establishes plan of care within 24 hours of admission or prior to start of procedure. Develops interdisciplinary individualized care plans in collaboration with the patient and healthcare team. Discusses patient's goals for pain management/control and incorporates into plan.

-IMPLEMENTS CARE. Provides age-appropriate and culturally sensitive care according to physician orders and patient's needs. Requests radiological, laboratory and other diagnostic tests as described by patient care protocols.
Demonstrates competence and knowledge in performance of basic nursing skills and those relative to area. Transcribes, verifies, administers and documents medication administration accurately (6 Rs).
Completes documentation of care delivered according to policy
Provides clear explanations/instructions to patients and families according to assessed needs.

-EVALUATES CARE DELIVERY. Recognizes age-specific responses to treatment. Evaluates and revises plan of care based on patient's expected outcomes. Ensures that patient education is understood and response is documented. Documents patient's response to care. Monitors care provided by health care team and provides feedback as needed.

-COORDINATES CARE WITH PHYSICIANS AND HEALTH CARE TEAM. Conveys pertinent findings from history, physical examination, test results to the physician promptly. Makes rounds on assigned patients with physicians and nursing staff. Assesses patient's needs daily. Participates with health care team to coordinate information for patient/family/significant others related to their overall status. Follows appropriate communication channels with recommendations and concerns.

-PROVIDES EDUCATION THROUGHOUT CONTINUUM OF CARE. Provides education to patient/families/significant others using age appropriate communication techniques. Explains routines and procedures to patient/families/significant others and responds to questions to increase understanding. Provides pre-operative education and post-operative follow-up as appropriate. Serves as a resource to patient/family/significant others and clinical staff. Provides or arranges inservices for new procedures, equipment, medications or protocols as needed.
REQUIREMENTS
This position is for a nurse coordinator in the Outpatient Lymphoma patient area. Requires a Bachelor's with Oncology/BMT experience. Case management and Oncology research experience would be a plus.

SKILL REQUIREMENTS:
Basic computer skills, 0 to 1 Year of Experience are required

EDUCATION REQUIREMENTS:
Bachelor's - Nursing or related health care field is required

LICENSURE REQUIREMENTS:
Licensed in the State of MO as a Registered Nurse is required.
American Heart Assn Healthcare Provider CPR/AED is required.
