External Job Description: RN/ Health Services Coordinator/Renal Case Manager ? Bilingual (English/Spanish) required RMS Disease Management, an affiliate of DaVita, Inc. and headquartered in Vernon Hills, IL is currently seeking a nephrology nurse to provide renal case management for our patients in Florida.
Minimum of 5 years experience in clinical nursing and three years in renal nursing.
Current RN license in Florida required and National Nephrology Nurse Certification preferred. Evidence of continuing education and CPR certification.

Exceptional prioritization and time management skills. A team player.
Solid computer skills including strong data entry skills to input notes into computer system
Degree required; BS preferred.

Able to work from home with approximately 60% travel (primarily same day, local area travel) to visit patients, attend physician visits, etc. Responsibilities will include but are not limited to -
Assist patients to achieve an optimal level of function by facilitating timely and appropriate health services. Identifies patient needs to include medical, social, emotion and financial concerns through medical record review, consultation with patient, family and health care team members. Enable patient to direct their care, self advocate, and make decisions whenever possible.
Apply nursing process (assessment, planning, intervention and evaluation) in managing caseload. Coordinate services to ESRD patients across the entire care continuum – including: physician office, emergency, outpatient dialysis, in patient acute/chronic, physician/provider/care team agencies and facilities, home care, ambulatory care, extended care facility, hospice.
Identify and redirect practice patterns to facilitate appropriate care and cost-effective outcomes. Establishes a mechanism for communicating with physician, provider and care team, and enters all notes/data into computer system for reporting purposes.
Functions autonomously and in collaboration with all members of the health care team to coordinate quality, cost effective care while minimizing fragmentation of the health care delivery system. The link between the patient, physician, provider and care terms, the community, and the Director of Health Service Coordination.
