Case Coordinator / Referral Development

MileStone Healthcare, the recognized leader in contract management of the full spectrum of post-acute care, seeks qualified candidates for Case Coordinator of a hospital-based Inpatient Acute Physical Rehabilitation program in Alhambra, California (Los Angeles area).

Responsibilities include identifying and responding to referral sources, pre-admission assessments, community education and monitoring QA and CQI program on the physical rehabilitation unit. Nursing background required; LVN or RN. Case management,
rehab marketing or nurse liaison experience ideal.
