Case Coordinator/ Designated Responder

The Case Coord-inator is responsible for the development of an Individualized Support Plan and related case coord-ination activities for the individuals assigned to their caseload. Assist with semi-annual quality reviews of individual's files. Responsible for direct supervision of consumers with a primary respons-ibility for responding to incidents of aggressive behavior on the part of consumers.

Qualification:
Strong, functional background in com-puter skills both in word processing and databases. BA degree from an accredited college or university in a be-havioral science preferred. 2 yrs accumulated college experience plus 2 yrs case management experience. Be in adequate physical health to perform the job tasks required which may include, but are not limited to being able to man-age the behaviors of individuals if needed, lift or transfer individ-uals, work in various environmental con-ditions, etc. Complete training in CPI, CPR, First Aid, and Medication Overview. Complete the CPI Train the Trainer the first six months of em-ployment.
