Here's a unique opportunity to join a respected name in the healthcare field and provide care for our ESRD (End Stage Renal Disease) patients. This is a newly added position due to growth of our program in the Tucson area. The chosen NP would enjoy lots of autonomy in a telecommuting role, and the chance to make a real difference in patients' lives.



Key responsibilities include:


Coordinating care, case-by-case, for our ESRD patients in collaboration with other clinical and facility personnel


Traveling to local facilities/dialysis centers to meet with patients and staff directly


Providing hands-on care for patients


Building and maintaining relationships with key facility personnel and colleagues


Arranging appointments with patients at your own discretion and documenting all care activities in our system

Job Requirements:

Only NPs with national certification in adult, acute, geriatric or family care will be considered for this role. Clinical experience as an NP is also needed. A background in ESRD, dialysis, home care, public health and/or health clinics is preferred. Spanish bilingual skills would be especially helpful in this role, but are not required. Basic PC proficiency is a must.
