Counselor- Service Provider

Using past experience working with Geriatric clients, provider obtains copy of the Pre-admission Screening finding from Area Manager. Obtains copy of Initial Resident Profile with resident specific goals and objectives from Area Manager. Provides direct mental health services to residents identified by the PASRR screening process. Maintains and encourages staff communication regarding plan of care and progress. Responsible to inform the Area Manager, and nursing home staff when aware of any remarkable conditions related to residents physical/mental condition or medication. Participate as a member of the multi-disciplinary treatment team at the nursing facility when necessary. Responsible for completing progress notes each time the resident is seen and addressing the following areas:
1.Accuracy of treatment planning
2.Clinical significance of identified target symptoms
3.Adequate rational for changes in treatment
4.Progress notes include symptoms, goals, interventions and response. Meets with Facility Contact person to discuss needs of resident, clinical findings and recommendations. Leaves one copy of the progress note with the facility. Forwards copy of Progress note to Atlanta office within five working days of the date of service.
Participates in monthly meetings with Area Manager and other clinical staff. Ensures confidentiality for each resident and that all residents are treated professionally and with dignity.
Seeks supervisory assistance and direction in order to maintain a high degree of efficiency, accuracy and proficiency.
Completes and forwards all inquiry, crisis incident, schedule, In-service report and other reports to Atlanta office in compliance with program guidelines. Provides five in-services per year to Nursing Facility staff. Conducts peer review activities according to established procedures and within established time-frames.
Provides on-going mentoring of new staff to ensure their performance and productivity as necessary via direct supervision
Participates in Individual treatment plan meetings and treatment plan review meeting when necessary
REQUIREMENTS
MHM Services, Inc. is currently seeking a part-time (20 hrs/wk) Masters level Counselor to become part of its PASRR Program that provides clinical services to nursing home residents throughout Georgia.

The ideal candidate will have a minimum of 3 years mental health and clinical management experience preferably in a geriatric setting to provide therapeutic support services to nursing home residents throughout Ware, Brantley, Pierce and some surrounding counties. Masters degree in Psychology/Counseling
Minimum of 2-3 years direct care to Geriatric patients in counseling/psychiatric nursing.
