RN Business Development Specialist/Hospital Referral Coordinator

Providence Sound HomeCare and Hospice in Southwest Washington State is currently recruiting full and part-time Business Development Specialist/Hospital Referral Coordinators. This position coordinates discharge planning in collaborative effort with hospital discharge planners. Other responsibilities may include assessment for Home Health and Hospice eligibility and admit to hospice care in the inpatient setting. Coordinators provide ongoing education and consultation for healthcare professionals within the hospital setting and are responsible for identifying customer needs and building positive customer relationships with hospital discharge planners, nursing staff and physicians. These responsibilities may extend to skilled nursing facilities.

REQUIREMENTS
Minimum Qualifications: Three years healthcare business development experience preferred. Three years home health/hospice or inpatient experience required. Home health/hospice discharge planning experience is a plus. Current RN Licensure in the State of Washington; BSN preferred. Must have the ability to function in an independent/interdependent role. Must have strong skills. Must have the ability to develop and nurture positive working relationships with diverse groups and individuals. Must be highly organized and have the ability to multitask. Must have computer skills. Valid Washington State driver's license along with an insured, reliable vehicle, CPR certification and Mantoux testing(TB),required.
