Home Care Specialist

Clinical Account Executive -ATTN RNs/ LPNs --A liaison role within the community for a large, national Home Health Company. Many Medicare referrals. Develops/ maintains contact healthcare professionals to provide information/ updates for Home Health care programs- determines potential clients' eligibility/need for home care, and coordinates care plan development. Participates in quality assessment/improvement activities and assists in implementing/ maintaining standardized procedures. Works under general supervision. Conducts pre-discharge hospital visits at the physicians' request to determine the need/eligibility for home care. Communicates and documents all necessary information to appropriate channels. Assists the Director in implementing/maintaining standardized operational processes to ensure compliance with internal controls and legal/regulatory agency requirements. Provides company sponsored continuing education programs for referral sources. Utliizes company collateral to promote business opportunities. Maintains market awareness and prepares competitive updates. Assists branch in DSO targets. Works with field sales positions to prioritize accounts in accordance with the market/territory sales plans.

REQUIREMENTS
QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions. EDUCATION and/or EXPERIENCE: Bachelor's Degree in Nursing or the equivalent, R.N. certification plus a minimum of three years clinical experience. Strong knowledge of governmental home health regulations, Medicare requirements and care plan development techniques required. Excellent organization, problem solving and communication skills also required.
