Assistant Case Manager

Options for Community Living, Inc. is committed to assisting individuals and families with special needs to develop their fullest potential for independent living. Residential and support services prepare participants for the demands and responsibilities of community life and promote housing permanency, health, safety and welfare. Program participants are involved in service planning and make informed choices. The organization’s actions are guided by principles of integrity, openness, accountability, respect for the individual, and the highest quality of care.

Options HIV/AIDS-related program provides intensive case management, housing, and outreach services to low-income individuals and families with or at risk HIV/AIDS.

Duties:
The Case Management Technician assists the case manager in the provision of intensive case management activities to support clients and their families in accessing needed services. The CMT makes phone calls to appropriate agencies to advocate for services, conducts home visits and community follow-up to monitor services and the client's status on a weekly basis. The CMT maintains relationships with service providers and referral sources and participates in case conferences. The CMT maintains up-to-date case records and billing activity logs on a daily basis.

Work Schedule:

Monday-Friday 8AM-4PM
8:30AM-4:30PM
or 9AM-5PM
REQUIREMENTS
Associates Degree in health and human services with one year of qualifying experience*ORHigh School diploma or GED with two years of qualifying experience*.
*Qualifying experience means: verifiable full or part-time case management or case work with the following populations:persons with HIV infection, and/or persons with a history of mental illness, homelessness, or chemical dependance.

Experience with families preferred. Spanish or Creole speaking a plus.

Applicants are required to have a valid driver's license, access to a car, and a safe driving record. The employee uses his or her own vehicle for transportation between sites and is reimbursed for mileage.
